OPHAC Hospital's Privacy Policy
In order to ensure the proper handling of your confidential information and in compliance with the "Personal Information Protection Act (Regulation 57 in 2004)", we use our own Confidential Information Protection Program.

The program is reviewed and audited more than once a year, and revisions are made whenever necessary. In addition, we are committed to dealing with any problems or concerns related to or resulting from the program with the highest efficiency possible.

Your confidential information is handled in accordance with seven clauses (please refer to "Clauses 1 - 7"). However, if any of the following exceptional circumstances occurs, our Confidential Information Control Manager will look into the matter personally and take full responsibility.

Exceptional circumstances :
  • Legal imperative
  • Emergency considered to be jeopardizing your life and/or personal assets
  • Unexpected and unavoidable circumstance which, if not treated as an exceptional circumstance, exerts a negative impact on anything related to the privacy policy and/or you
1. Security measures
  • Your confidential information is handled properly in accordance with our Confidential Information Protection Program
  • Our Confidential Information Control Manager, Trainer of Confidential Information Security, Enquiry & Complaints Manager, and Chief Auditor are appointed; and each carries out his/her responsibilities professionally at all times.
  • We make our best effort at all times to ensure that all our staff abide to the rules of our Confidential Information Protection Program in order to avoid any information leaks.
2. Collection of confidential information
  • We will never attempt to collect your confidential information without your consent and we will explain how your confidential information will be handled.
  • In the case where we are to collect your confidential information from a third party, we will confirm with you in advance whether you are aware of our intended purpose and obtain your consent if necessary.
3. Use of confidential information
  • We do not use your confidential information for anything other than the intended purpose, which you have given us consent for.
  • Every time we have to change our intended purpose, we will obtain your consent beforehand.
4. Storage of confidential information
  • Under constant supervision of the Confidential Information Control Manager, we maintain the highest standards possible with respect to storing your confidential information and preventing information leaks.
  • We secure data to minimize the chance of having any unauthorized access and loss, damage and/or falsification.
5. Disclosure of confidential information to third parties
  • We will never disclose your confidential information to any third partiy without prior notice and your consent.
6. Access to/making changes to/removal of your confidential information
  • Please contact our Confidential Information Enquiry & Complaints Department if you wish to inquire about, make changes to and/or remove your confidential information. For security reasons, we will need to verify your identify and document the reason(s) for your desired action(s).
